Job Description:
We are seeking a Case Manager who will be responsible for
providing case management to clients and their families. Provide
advocacy, support, and referral information and assist clients and
families to overcome barriers to become self-sufficient.

- 37.5 hour work week
Responsibilities include:
- Assist clients who have received Rental Assistance for Families
in Transition (RAFT) to ensure they remain stabilized in their
housing. Provide small stipends to assist them with obstacles in
maintaining housing.
- Complete intake and needs assessments with each participant,
which may involve a broad range of areas including housing, finance
& budgeting, employment, education, food & nutrition, school &
daycare, legal issues, health, parenting, and daily living
skills.
- Interview and evaluate clients to formulate Individualized
Service Plans based on Program requirements. Work closely with
clients to identify and build upon strengths and develop strategies
to address barriers and concerns identified through the assessment
process.
- Make individualized warm referrals based on need and
eligibility, including follow-up within 5-10 business days and both
3 and 6 months post referral.
- Establish, evaluate and monitor goals for clients linked to
measurable outcomes. Work collaboratively with interdisciplinary
team of collateral providers and partners to ensure coordination of
services.
- Maintain accurate case records by documenting all services
rendered including up to date case notes, telephone contact logs
and referrals. Enter required data into the agency databases with
reporting as required.
- Provide emergency response management to address any crises or
safety issues that may arise.
- Keep up-to-date knowledge of all local housing programs, their
requirements and availability.
- Develop and maintain a current list of available housing units
and landlords.
- Maintain a good understanding of housing issues such as
homelessness (family & individual); domestic violence,
foreclosures, rental assistance, prevention, evictions, housing
search and shut off notices, etc.
- Coordinate staff meetings with the team.
- Integrate a DE&I lens in all activities, encourage
colleagues to engage in DE&I work, and contribute to our

ongoing investment in a personal and professional journey to
understand identity and address unconscious bias, power, and
privilege.
Qualifications Include:
- HS Diploma or equivalency required, BA in human services,
psychology, sociology, or related field preferred.
- 3+ years' work experience with similar population.
- Working knowledge of Microsoft Office systems.
- Knowledge and experience working with low income, culturally
diverse, and at-risk families and individuals.
- Demonstrated knowledge of community resources available to
low-income individuals and families.
- Ability to work effectively with and meet the needs of people
from a diverse range of backgrounds, cultures, and identities,
including underserved communities and people from the different
racial groups, socioeconomic statuses, nationalities, and languages
that make up CTI's clientele.
- Excellent customer service, organizational, oral and written
communication skills.
- Ability to manage multiple priorities and work in a fast-paced
environment.
- Proficient computer skills required in basic computer software
and data collection systems.
- Excellent crisis management skills.
- Ability to maintain accurate records and confidentiality of
information.
- Excellent attention to detail.
- Bilingual preferred.
