Seeking a Virtual Care Navigator for a diverse,
community-based health care organization. The Virtual Care Navigator is a patient facing role, that
will support our virtual care program through patient outreach, education, tech
troubleshooting, and knowledge on all things virtual care. Virtual care
technology includes our patient portal, telehealth system (Doximity),
smartphones, text message outreach through our EMR and other patient engagement
tools. This role will do some CHW/case management work to assist patients apply
to programs that provide reduced internet costs, smartphones, or other support
for digital connection. This person will create patient education materials,
provide trainings, work closely with community health staff, & attend
community events to bring technical training to the community, and serve as a
patient navigator across the health center. This position is hybrid and
will require both onsite and remote work in order to better serve our patients. Bilingual in one of our top
languages required: Spanish, Portuguese, or Khmer. Education:










High school diploma or
equivalent, minimum of 2 years in customer/patient facing
Technology savvy, experience
providing technical support and comfortable troubleshooting in customer
facing role
Experience in healthcare setting;
EMR experience preferred
Ability to communicate technical
detail across levels of understanding & language
Discretion and culturally
sensitive attitude required
Experience working with diverse
communities and understanding of multi-cultural beliefs, practices and
cultures is a plus.
Multilingual preferred, bilingual
required in Spanish, Portuguese, or Khmer
