Seeking a Registered Nurse Care
Manager for a diverse, community-based health care organization. The Registered Nurse Care Manager
will help to provide care coordination and care
management within the Greater Lowell Behavioral Health Community Partner (BH
CP). The Nurse Care Manager is an integral member of the Care Team, who assists
with comprehensive assessments, developing a patient-centered treatment plan,
transitions of care and mediation reconciliation. Education:










Registered Nurse with experience
in the field of mental health and/or substance abuse.
Current Massachusetts RN license,
BLS, and minimum of 2yrs experience preferably with Behavioral health.
Experience with local resources,
navigating health, behavioral health and/or substance abuse treatment
systems.
Excellent communication,
documentation, time management and organizational skills.
Ability and desire work with a
diverse client population
Ability to manage difficult or
emotional situations; respond promptly to enrollee's needs.
