With a career at Lowell Community Health Center, you will be joining a dynamic team of passionate and talented people.The role of the Office Based Opiate Treatment (OBOT) Nurse includes providing an integrated and coordinated approach to client's enrolled in the OBOT Program. Advocating, facilitating and coordinating access to OBOT services by collaborating within the health center or collaborating with community partners to achieve efficient, accessible, appropriate and high quality OBOT services and care.Under general supervision, provides nursing and health care services to individuals and families in an outpatient primary care environment with assignments in the Office Based Opioid Treatment Program. Assist with telephone and face to face patient communications, physical exams, lab testing, patient education. Responds to patient needs and assures health care services for patients and families are reflective of Lowell Community Health Center mission and values. Incumbent works as part of the clinical team and assists in the continuity of medical/health care for patients.The assigned duties and responsibilities include, but are not limited to: Assists with provision of care that includes treatment, teaching and counseling. Recognizes and interprets symptoms of patient's conditions and reports to appropriate health care provider. Provides direct nursing services to patients and their families. These services include, but are not exclusive to: nursing assessment nursing intervention/referral health care education Assesses patients' needs based upon evaluation of current functioning and strengths, review of previous health/medical plan of care, and medications. This includes support system being utilized, vocational status, and identification of any special needs (i.e. physical, emotional, or substance abuse/psychiatric history) Able to Prepares patients for examination: completes history, reviews systems , and current physical health status takes B/P, weight, height, temperature, other measurements and/or vitals as appropriate to complete health assessment obtains urine specimens as appropriate complete laboratory reconciliation administers appropriate injections Maintains clinical logs for central area Develops a system for summarizing clinical and laboratory data for evaluation and planning purposes, and quality assurance Assists with the planning and implementation of educational programs for patients and families to increase awareness of relevant medical conditions, decrease risk-behaviors, and increase responsibility for self-care. Attends and participates in in-service education programs for staff, quality assurance programs, and risk-management programs Perform initial assessment and intake obtaining social, medical, mental health, substance use and medication history Reviews intake assessment with OBOT clinical team and establishes appropriate plan of care Implements relapse prevention counseling and pharmacotherapy Provides follow up care in person and via telephone when necessary Work with pharmacy, Behavioral Health clinicians, and OBOT clinical team in obtaining medication orders, prescriptions, refills, and communication as needed Assists PCP, care teams, and others in related prevention and education Collects patient data and uses established reporting tools Works with clinical care teams with integrating substance abuse services into primary care Development of community engagement plan to include all stakeholders ex: corrections, schools, shelters etc.Knowledge Current Massachusetts license as a Registered Nurse. General computer knowledge: Microsoft Outlook, Office, Excel. Use of Electronic Medical Record required.Experience Substance use disorder experience preferred Care management, or similar experience with special populations, preferred.We're an equal opportunity employer.
