About the role: FT HOURLY Day Shift, Community-Based Role

Community Health Worker to support our clients in Lynn, Lawrence,
Lowell, Haverhill, and Methuen. This full-time role will be
supporting the team 30+ hours a week, during business hours - no

Worker Responsibilities:Conduct on-site visits with each client and
caregiver at the qualified settingObtains a social history and
conducting a psycho-social evaluationConducts ongoing evaluations
of the AFC and/or GAFC settingSelect, train, evaluate, and
supervise AFC caregivers in conjunction with the registered nurse
and case managerParticipate in the development, implementation, and
ongoing review of the AFC plan of carePresents, plans, and
implements social service interventionsComplete a care-management
progress note corresponding with each on-site visit or encounter
and upon significant changeAssist with obtaining information and
accessing other healthcare and community servicesParticipate in
discharge and transition planning and implementationReview and
documenting the fire and safety procedures for the qualified
settingReview AFC caregiver task logs at a minimum of every 30 days
and report findings to the registered nurse and case managerReview
at least annually the suitability of the qualified setting.Provide
clients with ongoing support with ADL's and IADL'sProvide accurate
orientation to the participants of the AFC and/or GAFC
programCoordinate all social services with the personal care
teamPartake in both care plan creations and reviews, along with
discharge plansHas direct contact with the client and the client's
caregiver, registered nurse, and case managerTakes care of all
paperwork associated with admissions, referrals, visits, and
medical equipment/social referralsAssist participants with finding
community, financial and legal servicesReview and assess caregivers
clothing, equipment, supplies and other servicesPerforms any other
additional duties that may be requiredParticipates in weekly
clinical meetings in the Braintree officeReplacement visits for
Vitra RN & CM's on as needed basisConducts weekly calls to AFC
Caregivers to confirm compliance with program regulationsEducation,
Experience and Licensure Requirements:At least one years of
clinical experience in the care of elders or people with
disabilitiesPrevious knowledge of Medical TerminologyAdvanced
proficiency in Microsoft Office Suite and Google Suite (G
Suite)Valid driver's license with proof of insurance
requiredBilingual in English and secondary language preferred
