Job Description:
A company is looking for a Health Coach for the Healthy Outcomes
Team (Remote).
Key Responsibilities
Conduct inbound and outbound calls to members and providers to
facilitate healthcare services
Assess members' health status using motivational interviewing and
healthcare knowledge
Empower members to engage in their healthcare decisions and
overcome barriers to access
Required Qualifications
3-5 years of practice experience in relevant health fields
Experience in Managed Care or Medicare
1-3 years of experience with Microsoft Office
Associate degree or equivalent experience
Previous telephonic coaching or case management experience
preferred
