Element Care, a leader in providing tailored healthcare solutions for older adults, is seeking a compassionate and dedicated RN Home Care Coordinator. This position involves coordinating personalized care through the PACE program while ensuring patients continue to enjoy their independence at home. The ideal candidate is a skilled nurse who excels in managing multiple tasks and communicating effectively with diverse groups. Assess patients' healthcare needs through comprehensive assessments and coordinate with multidisciplinary teams to formulate individualized care plans. Implement care plans and monitor patients' progress, adjusting plans as necessary to meet their changing needs. Educate patients and families on health management, prevention strategies, and care options to promote optimal outcomes. Ensure compliance with all federal, state, and organizational guidelines relating to home health care and patient services. Maintain accurate documentation of patient care, condition, and progress through our electronic health records system. Provide emotional support and compassionate care to patients and their families. Strong clinical skills with a focus on geriatric care. Excellent organizational and time management skills. Proficient in using healthcare information systems and clinical software. Ability to work independently and as part of a team. Strong communication and interpersonal skills. Medical Insurance Dental Insurance Vision Insurance Current registered nurse (RN) license in the state of practice. Basic Life Support (BLS) certified or ability to obtain certification.
