Job Description:

Element Care seeks a compassionate and detail-oriented Enrollment Intake RN to join our team and play a crucial role in enhancing the lives of elderly patients through our PACE program. The incumbent will be responsible for the initial assessment and screening of potential patients, ensuring a smooth and efficient enrollment process into our comprehensive care services. Conduct initial health assessments and screenings on potential PACE program participants. Coordinate with interdisciplinary teams to collect patient history and current health data. Evaluate patient eligibility for the PACE program based on health status and other criteria. Provide detailed explanations of the PACE program benefits, services, and enrollment process to patients and their families. Collaborate with medical and administrative staff to facilitate effective patient enrollment and care planning. Maintain accurate records of patient interactions, assessments, and enrollment documentation in compliance with federal and state regulations. Participate in ongoing professional development and training to stay abreast of changes in regulations and best practices in patient care. Strong clinical skills with an emphasis on geriatric care Excellent communication and interpersonal skills Proficiency in electronic health record (EHR) systems Ability to work collaboratively in an interdisciplinary team Strong organizational and problem-solving skills Medical Insurance Dental Insurance Vision Insurance Valid Registered Nurse (RN) license in the state of practice CPR certification required Gerontological Nursing Certification (preferred but not required)
