This is an exceptional opportunity for a#RN Case Management Clinical Leader#to join our 3-time, Magnet-recognized, community-based hospital that delivers on our promise to provide#Complete Connected Care.## This is a full time, 40 hour per week position: 8:00am -4:30 pm, rotating on call weekends and holidays.# Job Summary:# The#RN Case Manager Clinical Leader of the Saints Campus#is responsible and accountable for the management of care for an assigned patient population by service line and across the continuum of care. The RN Case Manager works to achieve daily clinical, quality and cost outcomes by providing well-coordinated experiences for patients/families through the synchronization of care activities of multiple disciplines and negotiation with third party payers. o Systematically and continually collects data regarding the health status of the patient.

o Interviews and collects specific information and data from patients and families, contributes to the identification of specific physical, emotional, educational and cultural needs of those patients.

o Completes an admission assessment, to include appropriate documentation, within the time frames established by policy.

o Assess patients discharge and post-discharge needs and uses assessment as a basis for developing the discharge plan.

o Identifies barriers to efficient cost effective provision of care.

o Uses Intermural criteria to assign Admission Status to all patients, monitors status and changes as necessary.

o Uses appropriate evidenced-based assessment techniques and tools to continually gather information about the health status of the individual and family.

o Assess the safety of all patients at the unit level and follows established policies to provide a safe environment for all patients.

  • DEVELOPMENT OF PLAN OF CARE

o Analyzes assessment data, collaborating with physician, healthcare team and the patient/family in determining patient's Plan of Care

OUTCOME IDENTIFICATION

o Identifies expected outcomes individualized to the patient.

o Collaborates with physician, healthcare team and the patient/family in decision making and planning of care during hospitalization and in anticipation of discharge.

  • IMPLEMENTATION

o Implements the plan of care to achieve the identified expected outcomes for the patient.

o Utilizes community and other resources and systems to implement the plan facilitating referrals when appropriate.

o Provides education for the patients and families and documents according to policy.

o Coordinates activities related to financial/insurance issues including providing clinical updates to insurance companies, coordinating insurance appeals related to clinical issues and adhering to policies and procedures of Medicare and Medicaid.

o Reports frontline risk management and quality issues in a timely manner to the risk management coordinator and performance improvement liaison.

  • EVALUATION

o Systematically and regularly evaluates and documents the patient's response to interventions in order to determine progress towards achievement of expected outcomes.

  • PROFESSIONAL STANDARD

o Demonstrates through work performed, a thorough knowledge of and adherence to the evidenced based standards of nursing care, and the policies and procedures of the hospital and nursing division.

o Demonstrates understanding of Shared Governance by providing input, feedback and participation to assure autonomy competence and accountability.

o Teaches others to succeed by mentoring, in servicing, role modeling, presenting, orienting, and facilitating peer development.

o Seeks guidance and resources when necessary in the management of the plan of care from the healthcare team.

o The Staff Nurse evaluates his/her own nursing practice in relation to professional practice standards and evidence based care using the 360 evaluation process.

o Attends all mandatory educational programs and skills day,

o Actively participates in interdisciplinary rounds and collaborates with other health professionals.

  • QUALITY

o Uses the results of quality improvement activities, creativity and innovation to improve patient care.

o Assists in achieving unit quality scores which meet or exceed hospital established targets for nurse sensitive quality indicators as appropriate for the unit.

o Participates in unit specific Quality improvements including nurse sensitive indicators, national patient safety goals, core measures and other specific indicators being monitored. Verbalizes results and action taken to improve performance.

o Delivers quality care in a manner that preserves and protects patient autonomy, dignity, rights, confidentiality, privacy and security.
