MA Health Corps Care Management Specialist

Established in 1983, Epilepsy Foundation New England is a 501 c 3 nonprofit organization whose mission is to help people and families affected by epilepsy in New England. Epilepsy Foundation NE (EFNE) is an independent affiliate of Epilepsy Foundation of America whose mission is to lead the fight to overcome the challenges of living with epilepsy and to accelerate therapies to stop seizures, find cures, and save lives. We serve the approximately 104,000 people in Maine, Massachusetts, New Hampshire, and Rhode Island affected by epilepsy and the people who care for them. Twenty-six AmeriCorps Members will complete their service from August 2020-July 2021. They will be placed at one host site to support people living with and affected by epilepsy by mentoring and coaching; educating on the lifestyle of living with epilepsy; building a capacity for mentoring programs; program development and evaluation. All AmeriCorps Members will focus on volunteer recruitment and engagement efforts for the organization. They will work with each other and other Corps Members to expand statewide community service that benefits the field of mentoring in Massachusetts. Member Duties : Epilepsy Foundation New England is seeking one minimum time (300 hours) Care Management Specialist that will assist in creating best practices for 1:1 support for clients, otherwise known as Care Management. Care Management is a client/patient-centered approach to supporting people through the challenges they face while living with medical conditions. The Specialist will spend most of their time researching ways EFNE can best support their clients while assisting in our youth programs such as our Young Leaders Network.
