Job Description:

The Psychiatric Addiction Nurse Practitioner performs/supervises services in a designated area according to the policies, procedures, philosophy, and objectives of the department and hospital.##The professional nurse practices in accordance with the Massachusetts Nurse Practice Act, American Nurses Association Scope and Standards of Practice, Quality Caring Model and professional practice model at Lowell General Hospital.##Works cooperatively within department and other services to create a system of quality health care. # The Psychiatric Addiction Nurse Practitioner is a provider of direct adult addiction services for an assigned patient population across the continuum of care.# The Psychiatric-Addiction Nurse Practitioner will provide hospital-based Addiction Medicine Consults in both the ED and inpatient areas. The Psychiatric-Addiction Nurse Practitioner will develop the Addiction Medicine Consult Service and provide connections to the Bridge Clinic.# The role will also assist with coverage for other staff members--specifically the Psychiatric Mental Health Nurse Practitioners who provide psychiatric consultations on the inpatient service, the psychiatric nurse practitioners who provide psychiatric consultations and rounding in the ED, and the bridge clinic addiction nurse practictioner--when absent. #Assist in the development of cross-coverage schedule for the department with the goal to provide behavioral health and addiction treatment evaluation and treatment to Lowell General patients 7 days a week. The Psychiatric Addiction Nurse Practitioner synthesizes theoretical, scientific and clinical knowledge for the assessment and management of both health and illness states. Provides direct addiction and medication-assisted substance use disorder treatment services.# Incorporates the health promotion, health protection, disease prevention, and treatment focus of psychiatric-addiction nurse practitioner practice.# Performs a comprehensive physical, mental health, and substance use assessment.# Performs a comprehensive Substance Use Disorder evaluation that includes evaluation of mental status, current and past, substance use, medications, allergies, withdrawal symptoms, level of functioning, health behaviors, motivation, past history of addiction treatment, pertinent medical history, social history. Performs and accurately documents appropriate systems and symptom-focused physical examinations.# Demonstrates effective clinical interviewing skills that facilitate development of a therapeutic relationship. #################################### Services Provided by the Addiction Consult Service to include: The diagnosis of substance use disorders Initiation of evidence-based treatment (particularly medications) Linkage of patients to outpatient treatment Education on harm reduction and safety optimization # # Diagnosis of Health Status:### Engages in the diagnosis of substance use disorders, psychiatric disorders, and related health status.## This diagnostic process includes critical thinking, differential diagnosis, and the integration and interpretation of various forms of data.# Orders and interprets findings of relevant diagnostic and laboratory tests.# Differentiates psychiatric presentations of medical conditions from psychiatric disorders and arranges appropriate evaluation and follow-up.# Develops a differential diagnosis derived from the collection and synthesis of assessment data# Evaluates potential abuse, neglect, and risk of danger to self and others, such as suicide, homicide, and other self-injurious behaviors, and assist patients and families in security the least restrictive environment for ensuring safety####################################### Plan of Care and Implementation of Treatment:# Assist the patient to achieve an optimal state of health.# Develops a treatment plan for substance use disorders based on evidence-based standards of care and practice guidelines.# Plans care to minimize the development of complications and promote function and quality of life using treatment modalities such as, but not limited to, psychotherapy and psychopharmacology.# Prescribes psychotropic and related medications based on clinical indicators of a patient#s status, including results of diagnostic and lab tests as appropriate, to treat symptoms of psychiatric disorders and improve functional health status.# Manages psychiatric emergencies by determining the level of risk and initiating and coordinating effective emergency care.# Coordinates the transition of patients and families among mental health care settings, general health care settings, and community agencies to provide continuity of care and support for the patient, family, and other health care providers. Identifies, measures, and monitors clinical and related behavioral outcomes to determine the effectiveness and appropriateness of the plan of care.## Makes appropriate referrals to other health care professionals and community resources for individuals and families.# Applies ethical and legal principles to the treatment of patients with addiction, mental health and psychiatric disorders.# Ensures patient safety through the appropriate prescription and management of pharmacologic and non-pharmacologic interventions. ################################################ Nurse Practitioner-Patient Relationship:# Demonstrate the personal, collegial, and collaborative approach that enhances the psychiatric addiction nurse practitioner#s effectiveness of patient care.# Speaks to the critical importance of interpersonal transactions as they relate to therapeutic patient outcomes.## Maintains a therapeutic relationship over time###with individuals, groups, and families to influence negotiated outcomes.## Monitors own emotional reaction and behavioral responses to others and use this self-awareness to enhance the therapeutic relationship.#### Uses the therapeutic relationship to promote positive clinical outcomes.# Identifies and maintains professional boundaries to preserve the integrity of the therapeutic process.# Analyzes the impact of duty to report and other advocacy actions on the therapeutic relationship. ################################################ Teaching-Coaching Function:### Imparts knowledge and associated skills to patients.# Interprets and individualizes therapies through the activities such as advocacy, modeling, and tutoring.# Teaches patients and significant others about intended effects and potential adverse effects of treatment options.#### Provides psycho education to individuals, families, and groups to promote knowledge, understanding, and effective management of addiction, mental health and psychiatric disorders.#### Considers readiness to improve self-care and healthy behavior when teaching patients with addiction, mental health, and psychiatric disorders.## # Professional Role:## The psychiatric-addiction nurse practitioner demonstrates a commitment to the implementation, preservation, and evolution of the psychiatric-addiction nurse practitioner role.# The psychiatric-addiction nurse practitioner implements critical thinking and builds collaborative, interdisciplinary relationships to provide optimal care to the patient.## Advocates for the advanced practice psychiatric-addiction nurse#s role to other health care providers; community, state, and federal agencies; and the public.#### Upholds ethical and legal standards related to the provision of mental health care.# Recognizes the importance of lifelong learning to be knowledgeable of relevant research and advances in clinical practice.########################################### # Cultural Competence:## Delivers patient care with respect to cultural and spiritual beliefs, and makes health care resources available to patients from diverse cultures.
